Health Screen Forms

• First Year
  • Any athlete who has never competed at Long Beach City College (you may have previously competed at the collegiate level at another school).
• Second Year
  • Any athlete who competed for Long Beach City College the previous year.
• Note: If you have any break in your LBCC athletic eligibility (e.g. medical redshirt, academically ineligible, etc.), you must complete a "First Year" health screen.
